How do I schedule an appointment?

Simply call the location you wish to be seen at.  If you are calling to schedule in the Upland location, you will contact the Muncie office to schedule your appointment.  Please confirm with the scheduler your desired location.  If there is a particular physician you would like to see, please let the appointment scheduler know.  If you have no preference you will see the first available physician who sees your type of problem.

If I am referred to a CIO physician what do I need to do prior to the appointment?

You must obtain any previous X-Rays, MRI's, CT, Operative Reports and any other notes associated with your current injury.

If I need surgery what do I need to do?

You will see a surgery scheduler the day the decision for surgery is decided upon.

If medical clearance is needed prior to surgery, the surgery scheduler will help you get the tests you need done.  If you do not need medical clearance tests done, the surgery scheduler will set up your appointment at that visit.

If you have any questions regarding surgery scheduling, please call the appropriate office and ask to speak with the surgery scheduler.

If I need my x-rays, how do I get them?

Please call the office where the x-rays were taken and ask to speak with the x-ray records clerk.  You will need to fill out an Authorization to Release Information Form.  Click Here to access this form.

How do I request medication refills if necessary?

Central Indiana Orthopedics medication call-in hours are Monday through Thursday 8am to 5pm and Fridays 8am to 3pm.  We do not refill narcotic prescriptions after hours or on weekends.

If I need a copy of my medical records, how do I get them?

Please call the apropriate office and ask for the medical records department.

You will need to fill out an Authorization to Release Information Form.  Click Here to access this form.  There is a charge for copies of medical records.

How do I get disability forms completed?

Send or drop off your disability forms to the appropriate office and allow 7 to 10 days for completion.  There is a $15.00 charge for disability forms.  Once the form is completed, you will be notified and can pick it up, have it faxed, or mailed when the payment is received.
